Community Health and Hospital Community Benefit

Community Benefit Reporting Workbook

This Excel workbook consolidates five hospitals' community benefit activity logs into one dataset and rolls it up to the lines of IRS Form 990, Schedule H. Every total on the summary ties back through formulas to the row a hospital reported.

Sample data. All names, organizations, and figures are fictional.

About this sample

The workbook follows a five-step workflow (stack, map, confirm, cost, review), and each step has its own sheet. Crosswalks map 150 staff title variants to 22 standard positions and each hospital's category labels to Schedule H lines, staff time is valued at loaded hourly rates with an indirect cost rate, and a formula-based match key flags duplicate rows for a reviewer decision, including activities that two hospitals both reported.

The SUMMARY sheet shows the Part I, line 7 table and the Part II community building table for the system or any one hospital, with eleven checks that turn green when the totals tie to the detail. The file also includes an instructions sheet for hospital finance staff and a QA log of every issue found and resolved. The sample is built on a fictional five-hospital system, and its line labels follow the 2025 Schedule H.
